Antioch, IL Antioch, IL Part-time PRN Part-time PRN $45 an hour $45 an hour Job description Center of Illinois Home Healthcare is looking for Home Health LPN to provide services in Antioch, IL and surrounding area in patients homes.
You will also be asked to admit and visit patients in the surrounding towns.

Your
Responsibilities:
Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care Responsible for timely completion and synchronization of visits within the patient home via device (EMR) Contributes to the assessment of the health status of individuals or groups.
Participates in the development and modification of the strategy of care Implements the appropriate aspects of the strategy of care and participates in the evaluation of responses to the strategy of care Required skills and
Qualifications:
Graduate of an accredited school of professional nursing Licensed as an LPN in the state of IL One year related experience, including recent Medicare home health experience preferred must have a reliable car w/current insurance coverage and a current driver's license Experience with Electronic Medical Records (EMR) devices preferred but not required Job Type:
Part time This is a pay per visit position not hourly Pay:
$45.
00 per visit not hourly.
